What to ask when ranking a co-worker?

I am making a survey up and need questions for ask employees to rank their co-workers (wait staff). example - 1st to help out, on time, team player - etc.

What to ask when ranking a co-worker?
DON"T DO IT!!!!!!!!!!!!

even with the best of intentions and planning this seldom works and almost always causes more problems then it resolves.

having employees evaluate each others performance always ends up being a popularity contest or the results are not guarded, become general knowledge and cause friction among the staff.

it is managements job to evaluate employees performance and to address any issues that are found by preparing a plan for improvement which must include input from the employee being evaluated.

DO NOT GO DOWN THIS ROAD YOU WILL REGRET IT,

YOU WILL CAUSE MORE PROBLEMS THEN YOU EVER HOPED TO SOLVE.

EDIT:

for another set of eyes use a secret shopper. you do not need to pay for this service but rather ask someone you know whom you trust to go in and evaluate their experience, the management and the staff when you are not there. give them some idea of things to look for and how to evaluate the visit or a particular employee to observe. they need to know that this is a tool to help you and to be honest with their evaluation good or bad. they can provide for you a wealth of information and all it cost you is a complimentary meal and it could help prevent some of that "high-school drama". a secret shopper program in the form of a paid service or if you do it yourself on occaision can be a valuable tool if it is used as a learning tool to gauge a moment in time and not used to "beat-up" your staff. you seem to be interested in finding tools that will help you so i don't think this would be an issue in your circumstance.
Reply:Why on earth would you want your employees to rank their co-workers? What could you possibly gain? If you're management, you should be ranking them, not the employees. What you could do, though, is ask for people to write down any suggestions they might have to improve the business. That would be a bit more on the positive side compared to what you're wanting to do.
